Salary: $42,976 - $54,440 per annum plus 25 percent COLA Region: North Central Organization: Bassett Army Community Hospital Community Mental Health, SVC-FWA MCUC-CM Duty Station: Fort Wainwright, AK Area of Consideration: Open to all U.S. citizens. Expanded Definitions Duties: Incumbent serves as a Clinical Nurse Specialist for the Department of Psychiatry, Bassett Army Community Hospital. The primary responsibility of the position is to facilitate access of beneficiary patients to psychiatric services, to facilitate movement between appropriate levels of care throughout the direct care and TRICARE Network facilities, to promote continuity of care between Primary Care Managers and Department of Psychiatry clinicians, to assist in coordination of medical and social resources as they support individual patient care, and to support overall patient health through direct clinical service delivery and mental health promotion. MAJOR DUTIES: 1. Responsible for triaging of psychiatric patients presenting telephonically or in-person, and at varying levels of urgency (emergent to routine). Serves as primary screening provider of new patients seeking entry into the Outpatient Psychiatry Service of the Behavioral Health Clinic. 2. Based upon the results of psychiatric nurse triaging, the incumbent facilitates entry into the appropriate level of care, at the appropriate level of clinical urgency and within TRICARE access standards. 3. The incumbent works in close collaboration with physicians and other healthcare personnel in performing initial psychiatric emergency screenings within the Health Clinic and MEDDAC Emergency Room. Works with chief of alcohol treatment program and TRICARE case managers to initiate and manage inpatient or residential placement and transportation of beneficiaries requiring specialized alcohol or drug rehabilitation programs; facilitates aftercare with available community resources. Applies knowledge of psychiatric nursing practice to identify any life-threatening conditions requiring immediate psychiatric intervention. 4. The incumbent participates in case management and joint planning with Health Care Providers, Health Benefits Advisors, Health Care Finders, Discharge Planners, Ambulatory and Inpatient Case Managers, and Utilization Review officials in military and civilian health care facilities to identify beneficiaries with long-term mental health care needs and/or high risk, high cost diagnoses. 5. The incumbent participates as an active member of the treatment team in those cases that he/she case manages. This includes serving as a point of contact between the patient and the treatment team, triaging and answering treatment questions and appropriately referring questions to the primary clinician, when needed.
