Job Description:  Category: Nursing//RN, Facility:Banner Thunderbird Medical Center ,Location: AZ-Glendale (Banner Thunderbird), Department:Periop Svcs, Shift: Days; 36 to 40/hrs per week. ACLS certification will be required within six months of starting position. 1 year OR experience required. JOB SUMMARY: Assesses, plans, implements, evaluates and documents nursing care of patients in accordance with facility and departmental policies and in accordance with standards of professional nursing practice. This position is accountable for the quality of nursing services delivered by self or others who are under his/her direction. The Registered Nurse utilizes specialized knowledge, judgment, and nursing skills necessary to assess data and plan, provide and evaluate care appropriate to the physical and developmental age of patients on his or her assigned unit. Responsibilities: ESSENTIAL FUNCTIONS: 1. Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. a. Reviews patient History & Physical with patient/family, assures completion within appropriate timeframe. b. Reviews available information obtained by other health care team members. c. Reviews diagnostics and laboratory data and reports abnormal results to the physician(s) and other appropriate caregivers. d. Completes assessment and reassessments according to patient need and as outlined in policy and Standards of Care. 2. Formulates a plan of care, including the discharge plan, utilizing assessment data, patient, family and health team input. a. Initiates a plan of care based on patient-specific needs, assessment data and the medical plan of care. b. Goals for patient are realistic, measurable and developed in conjunction with the patient/family. c. Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient in the plan of are. d. Plans care in collaboration with members of the multidisciplinary team. e. Reviews and revises the plan of care to reflect changing patient needs based on evaluation of the patient’s status. 3. Implements the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team. a. Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements. b. Delegates appropriately, and provides nursing supervision in the provision of care to patients by other licensed nurses and other personnel. c. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for whom care is provided. 4. Evaluates the patient’s and family’s response to care and teaching, and revise the plan of care as needed. a. Evaluates patient progress towards goals and expected outcomes in collaboration with other health care team members. b. Evaluates patient’s response and the effectiveness of patient teaching. 5. Documents assessment, planning, implementation and evaluation in the patient record. a. Documentation is legible, timely and in accordance with policy. b. Documentation reflects objective/subjective data, nursing interventions and patient’s response to treatment. c. Notes physician orders accurately and in a timely manner. Follows safety standards in all aspects of performance of the above functions, reports/removes unsafe equipment and attends safety education sessions. 6. Follows safety standards in all aspects of performance of the above essential functions. a. Follows standard precautions in the provision of patient care and implements other infection control measures as necessary. b. Utilizes equipment according to manufacturer's recommendations and hospital policy. c. Reports/documents unexpected occurrences involving patients and others as appropriate. d. Follows hospital and unit specific emergency and disaster preparedness plan. e. Actively participates in processes that support national patient safety goals. 7. Promotes professional development within delivery of care. a. Maintains clinical performace competencies appropriate to the area in which they work as deomonstrated through annual validations. b. Incorporates, supports, and participates in concepts of shared leadership at both departmental and facility level. c. utilizes evidence based practice or research within clinical practice. d. Actively participates in quality improvement process. Actively participates in performance measures of national nursing quality indicators. e. Accountable for patient flow to enhance throughput within department. f. Interacts and participates in the education/orientation of hosiptal personnel, patients, students, families and visitors. Promotes/supports growth of others therough precepting and mentoring. Follows safety standards in all aspects of performance of the above functions, reports/removes unsafe equipment and attends safety education sessions. PHYSICAL/MENTAL DEMANDS/ ENVIRONMENT: Must be able to speak, read and write in English; calculate basic math and algebraic equations; see, hear, use fine motor skills, eye-hand coordination, and tactile abilities. Stand, walk, bend, squat, reach and stretch frequently; physical agility to respond in a quick manner and appropriately to respond to unexpected patient and family safety needs; perform proper body mechanics and have the ability to assist patients in ambulating, transferring in and out of bed, chair or wheelchair. Lift 0-50 lbs. Must be able to use critical thinking and problem-solving skills and analyze complex data. Environment is a patient care area and associated treatment and procedure rooms that may require maneuvering equipment in tight spaces. May be required to work in highly stressful and critical situations requiring quick thinking and coordination of a variety of activities at once. There is potential exposure to hazardous substances. Must demonstrate proper body mechanics. EQUIPMENT USED: Complex technical invasive and non-invasive patient monitoring equipment specific to the clinical area and the age of the patients served. Computers, telephone, fax, copiers, printers and other communication devices. MARGINAL/ADDITIONAL FUNCTIONS: 1. Professional Development: a. Understands and facilitates Nursing Resource Management/Staffing System process. b. Maintains awareness of current unit, campus and system information updates. c. Completes mandatory educational requirements. d. Supports and participates in shared leadership process. e. Recognizes own learning needs and seeks opportunities for improvement. f. Directs concerns of clinical practice/operations through appropriate channels. g. Interacts and participates in the education/orientation of hospital personnel, patients, students, families and visitors. May function as preceptor/mentor. 2. Performs other duties as required. SUPERVISORY RESPONSIBILITIES: Directly Reporting: Provides direction and supervision to licensed and non-licensed personnel in the activities necessary to provide quality care and services.
