Per Diem hours per week. &
Open 2/27/2006.
$12.21 per hour minimum. & $14.65 per hour midpoint. & $17.08 per hour maximum.
Must be a graduate of an accredited school of practical nursing with current Oregon license. Must have knowledge of patient care procedures, problem solving skills and excellent interpersonal skills. LPN position to work with the Diabetes Wellness Program. Primary responsibility is care management with patients and coordiation of care with provider teams. Registry work also involved as well as report monitoring. Excellent opportunity for increased hours with cross training to other job duties a possibility and increased hours
