Category: Rehabilitation Services//Other, Facility:Community Hospital - Torrington ,Location: WY-Torrington, Department:Physical Therapy, The Licensed Physical Therapist evaluates, develops a plan and administers treatment programs for patients of all ages to optimize physical function, minimize pain, promote independence and provide education for safety at home, work and in the community. The Licensed Physical Therapist is accountable for the on-going data collection and documentation of all components of care delivered to customers in BH. Responsibilities: 1. Member of the Profession: Takes the initiative to obtain and present training/education to ensure individual skill levels meet the demands for areas of responsibility. Demonstrates willingness to cross-train in other health care environments. To take Physical Therapy expertise into homes, schools, nursing homes, or wherever directed for the purpose of administering treatments and direct and supervise patient and /or family and aides in homes or facilities. Demonstrates time management for patient care and paperwork. Maintains an entrepreneurial spirit in developing new or expanded service/program opportunities. 2. Provider: Assures quality patient care by performing accurate evaluations, developing a plan to address deficits/issues and implementing therapeutic interventions. Demonstrates proficient performance of interventions according to the scope of care. Documents all interactions and procedures according to department policies. Conforms to established productivity standards. 3. Advocate: Identifies and meets/exceeds customers’ needs by providing quality services. Complies with all infection control and blood and body fluid policies. Participates in PI programs to address issues that impact patient care. 4. Manager: Provides training, work direction and works closely with paraprofessional rehab staff, PTA’s, CMT’s, students and clerical support to ensure proper performance of the assigned tasks. In the absence of the Director, may assume the responsibilities of the Physical Therapy Department, including scheduling, staffing, and other responsibilities as needed. Provides oversight of all assigned direct patient care activities. 5. Educator: Instructs the patient/family and other health care team members in the techniques most effective for optimizing function and safety. Participates in community wellness and outreach activities. Qualifications: ·Education/Experience: Graduate of an accredited school of physical therapy. ·Other qualifications/certificate/license: Current Wyoming Physical Therapy License and current Basic Cardiac Life Support Certification.
