Category: Nursing//Leadership, Facility:Washakie Medical Center ,Location: WY-Worland, Department:Care Coordination, Full-time. Days 8:00 a.m. to 4:30 p.m. Salary range $16.89 to $27.03.
Responsibilities: To assess, plan, implement, coordinate, monitor, and evaluate options and services to meet an individual’s health needs.
Qualifications: Graduate of approved school of nursing with BSN preferred. Current nursing experience in acute, community or care coorination role. Knowledge of clinical standards and outcomes. Current Wyoming Nursing License and certification in BLS. Certification in Case Management related field and/or Utilization Management preferred.
