Description:*** Case Manager-Substance Abuse IOP [ 10320 ]***Case Manager- Substance Abuse IOP (10320)Part-Time-20 hours per week, Tuesday-Thursday, 1:00p-9p.Description:Develops and implements treatment plan for stabilization, mobilization and disposition appropriate for patient care within 72 hours of patient admission. Conducts utilization and review to ensure clients receive authorization from payer source for treatment. Facilitates psycho educational groups on addiction and recovery as part of core programming. Participates in marketing and business development initiatives to insure viability of program.Documents case management activities in patient’s medical records as required. Ensures quality of care for patient by maintaining clinical focus on the restoration of the patient to physical psychological safety. Facilitates interdisciplinary case conferences detailing treatment plan and patient progress.Consults and acts as liaison with all other members of the treatment team to mobilize patient resources insuring effective transition.Provides educational information to patients and their families in individual and group settings.Provides patient with information regarding community resources. Provides aftercare planning and disposition. Ensures compliance with applicable regulatory guidelines and requirements. Develops community resources and educates other department members of those resources. Participates in patient centered departmental meetings as well as non-departmental meetings and ongoing hospital committees as requested. Maintains clinical skills through continuing education. Represents adherence to mission philosophy, vision and objectives of the hospital and the department of Behavioral Health. Completes all required competencies and maintains all health standards as required by hospital in a timely manner.Responds to all patient emergency situations when requested.MINIMUM REQUIRED QUALIFICATIONSMasters degree in mental health field. LCSW, LMFT, LPC, RN preferred.Or Bachelors degree in mental health field with CAC III license and 5 years relevant experience.Graduate work including field placement within the mental health and/or recovery field.Two years experience in chemical dependency clinical care and case management.Certification in Alcohol Counseling or experience with Addictions Counseling.
