The best people, the best hospitals - they go together.Our PlaceThe Clinical Pharmacist I opening is in our Pharmacy Department at Exempla Saint Joseph Hospital. The Pharmacy Department operates the main Pharmacy, located on the ground floor of the hospital, and three fixed satellite pharmacies: Operating Room in the surgical suite; ICU Satellite pharmacy on the 6th floor; and the NICU Satellite pharmacy located on the 2nd floor in the NICU of the Mother and Baby Unit. In addition, the Pharmacy Department operates four mobile decentralized units to serve various floors and units within the hospital. Our medication distribution is accomplished by unit-based, automated dispensing devices and by centralized/decentralized unit dose distribution in combination with an IV admixture program. The Pharmacy Department is fully computerized and provides comprehensive pharmaceutical services 24/7.Our clinical programs include pharmacokinetic monitoring, an anticoagulation service, therapeutic substitution, drug information, IV-to-PO conversion, renal dosing, antibiotic dosing, an investigational drug service and a pneumococcal vaccine program. Pharmacists also participate in the development, implementation, and monitoring of clinical practice guidelines, drug use evaluation criteria, and performance improvement activities.The Pharmacy Department staff consists of 60 FTEs of which about 40 are pharmacists with the remainder a combination of technicians and support personnel. The structure includes a director, two managers, three clinical pharmacy specialists (CPS) and two pharmacy practice residents. The role of the CPS includes development of clinical programs, participation in formulary functions, conducting drug use evaluations, clinical practice and support of staff clinical pharmacists. A CPS is available via pager 24 x 7 to assist other pharmacists with clinical issues.Your PositionTo fulfill this important role as a Clinical Pharmacist I in the Pharmacy Department, you'll coordinate, develop, maintain, and provide pharmacy services to patients throughout the Hospital and ambulatory clinics. As a Clinical Pharmacist I, you'll prepare and dispense medication orders according to established policies, procedures, and protocols. You'll serve as a team leader through the supervision of interns, technicians, and support personnel as required. As a Clinical Pharmacist I, you'll ensure safe, ap propriate, cost effective medication therapies for patients, provide medication related clinical consultation and clarification to other health professionals, and develop and participate in unit-based activities designed to measure and improve the medication use process.
