Looking to utilize your clinical skills, but would prefer to move off of the floor? This is a fantastic opportunity for an RN with at least 5 years of previous Hospital Clinical Experience. Regional Referal centers are looking for RN's to work as Patient Care Coordinators -Patient Advocates. Position involves working with patient's Primary Care Providers to help case manage clients in a hospital setting. You'll also interact with Discharge Planners making sure that the patient's transition from the hospital to the home setting is coordinated and appropriately planned.
